Thank you for your interest in the Eaton Brain Improvement Centre’s assessment questionnaire.
The questionnaire takes approximately 15 to 20 minutes to complete. There are 100 questions to answer. Each question relates to different cognitive functions of the brain such as memory, reasoning, comprehension and processing tasks. The four possibilities for each question include; Never, Seldom, Often and Always.
It is important to read each question carefully and answer with complete honesty. Please take your time and reflect on each question. Some of the questions may make you think for the first time about a specific activity in your life, and thus it could take some time to consider the most appropriate answer.
